registrationWhen you request to register with the practice, and you are accepted, you will be assigned to a named doctor as arranged by the practice. This helps to spread the workload of the GP’s evenly to avoid undue pressure on any one GP. If you wish to particularly register with a named GP, please request this as you ask to register.

The practice has a responsibility to ensure that patients requesting to join the practice are entitled to free primary health care. Patients who are not normally resident in the UK are only normally entitled to emergency treatment, or for treatment for a condition that was not current prior to their visit, unless they have certain recognised papers.

Patients not entitled to free primary medical care will be charged. Some countries have reciprocal arrangements, check with reception. Asylum Seekers or refugees will have their relevant  Registration Cards to show they are entitled to free treatment and registration.

After you have been accepted to the Practice by your Doctor, you will be asked to complete the following forms as soon as possible before you can be officially registered or seen:

  • A practice registration form
  • A New Patient Health Questionnaire, which will be seen by the doctor who may request to see you, or you may be required to see the practice nurse
  • You will also be asked to complete a form asking for your ethnicity and your main language, this is a requirement for the government.
  • You will also be asked to complete a form asking about your drinking and smoking habits

If your doctor wishes to see you as a new patient, please bring all your current medication with you, including any inhalers, and any records that will be of use to the doctor, ie vaccination information, particularly for those under 18 years.
